They always get into a big knot in the back, under my real hair can anyone help me, is there a trick out there?

What kind of extensions?? Bonded strand-by-strand?
Didn't your stylist give you instructions on how to care for them? If not, find a new stylist!!
If they are the bonded/fused kind:
You have to pull them apart on a daily basis. Your head sheds hundreds of hairs daily, and those hairs that are still attached to the bond will get tangled with your other hair and eventually get matted if you don't pull them apart. You are also supposed to go to the stylist every month or two to get them tidied up (she'll detangle it and cut some of the shedded hairs off).

Hair extensions... they can look lovely, but they do harm the hair and fall out easily. What kind do you have (adhesive or)? How they are placed in your hair may be the reason they are getting messed up.
